Insurance

Our office accepts most major medical PPO insurance policies, however: we are not preferred providers with these organizations.

As a courtesy to you, we will request prior authorization and submit claims to your insurance company for allowed procedures and services.

Financial

For your convenience, our office accepts personal checks, Visa, Master Card, American Express and Discover. Payment is collected at the time the service is rendered for your consultation, or prior to any surgical procedure or other planned treatment.

If you are unable to pay your account timely, please contact our office to discuss options at 805-682-8462. We encourage open communication and will do our best to work with you.

Please remember that you are responsible for any charges incurred on your account, regardless of insurance coverage.
